About the Role:

We are seeking a skilled and experienced Social Worker to join our Hospital Discharge Assessment Team. You will play a crucial role in facilitating timely and safe discharges from Kings Mill Hospital.

Key Responsibilities:

Conduct comprehensive assessments of patients' social and care needs upon hospital admission. Develop and implement discharge plans, coordinating with multidisciplinary teams to ensure seamless transitions of care. Liaise with community services, care providers, and other relevant agencies to secure appropriate post-discharge support. Advocate for patients' needs and rights, ensuring they receive the best possible care. Participate in a rota system to cover the Transfer of Care Hub at Kings Mill Hospital. Maintain accurate and up-to-date records. Promote effective communication and collaboration with healthcare professionals, patients, and families. Essential Requirements:

Social Work Degree (or equivalent) HCPC Registration Significant experience in a hospital or community setting Strong assessment and care planning skills Excellent communication and interpersonal skills Ability to work independently and as part of a team Flexibility to work shifts, including weekends and evenings Desirable Requirements:

Experience in discharge planning and complex case management Knowledge of local health and social care services Ability to manage a demanding workload Benefits:
