We are currently recruiting for a dedicated and compassionate team committed to providing high-quality social care services to adults in the community. This will be working within an Adults Locality Team playing a crucial role in supporting individuals to live independently and with dignity. We are seeking a Qualified Social Worker to join this Adults Locality Team on a permanent basis. The successful candidate will be responsible for assessing, planning, and implementing care plans for adults with a range of needs. You will work collaboratively with other professionals to ensure the best outcomes for our service users.

Your new role

Conduct comprehensive assessments of service users' needs.Develop and implement personalised care plans.Provide support and advocacy for service users.Work in partnership with other agencies and professionals.Maintain accurate and up-to-date records.Ensure compliance with relevant legislation and policies.

What you'll need to succeed

Degree in Social Work and registration with Social Work England.Proven experience in adult social care.Strong assessment and care planning skills.Excellent communication and interpersonal skills.Ability to work independently and as part of a team.Commitment to continuous professional development.Must be able to drive and have access to a car due to the rural location.
