Co-ordinate the client assessment process, ensuring involvement of the client in identifying needs and preferences, and integration of all information into the care plan. Ensure that all relevant information in respect of each client is communicated as appropriate to achieve delivery of person-centered approach. Champion ANA Nursing’s vision and values, adopting an open management style supporting person-centered care. Complete all allocated one-to-one meetings and yearly performance reviews for the carers in the Company. To ensure standards and processes are followed at all times. Regularly communicate to family members the needs and progress of the client. Report staff absence to the Care Manager. Conduct and oversee effective handovers between all shifts, ensuring that the management are kept fully informed at all times with regards to applicable areas.

Provide daily coaching and support to Care Assistants as needed, leading by example at all times. Develop and execute daily programming for clients focusing on person centered activities
