Fernbank Healthcare have partnered with a PCN on a strategic appointment of a Advanced Nurse Practitioner to work a temporary contract until March 2025 with the possibility of an extension.

Duties:

Assess, diagnose, plan, implement and evaluate treatment interventions in patients presenting with minor illnesses and long-term conditions (as appropriate) Diagnose and manage acute and some chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan. Prescribe medications for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national practice protocols within scope of practice. Patient support for compliance and adherence to prescribed treatments. Prioritize health problems and intervene appropriately to assist patients in complex urgent or emergency situations including initiation of effective emergency care. Support and empower patients to adopt health promotion strategies for enhancing healthy lifestyles. Experience of working within a GP practice is ideal.
