Hunter Mental Health is currently working alongside an excellent Adults Hospital Discharge and Planning Team and assisting in their search for a locum Social Worker.

Key details of the position:

Job Title - Registered Social Worker (MUST have 2 years post AYSE experience)Location - BathDuration - 3 months on goingSchedule - Monday to Friday, 9am to 5pmStart Date - November 2024Rate of pay - GBP34.38 - GBP36.50ph (Umbrella) You will be responsible for:

assessing the needs of patients and their carersarranging appropriate care packages and servicesliaising with other professionals and agenciesensuring a safe and timely dischargecarry out assessments and reviews in residential and nursing homes, as well as people s own homeswork to prevent re-admission to hospital and to establish and plan to meet longer term outcomes. Requirements and qualifications required for this role:

Current and valid registration with the SWEProven experience within a Social Working roleStrong communication and interpersonal skillsAbility to work independently and as part of a multidisciplinary teamCommitment to providing high-quality patient careCar driver to travel across multiple sitesFully on site and hybrid/remote working to be discussed during interview (team can offer up to 2-3 days remote)
