Responsibilities: The team is made up of experienced social workers and social care assessors who work alongside doctors, nurses, occupational therapists, physiotherapists, mental health services Work with a multidisciplinary team to facilitate appropriate, safe and timely hospital discharges for those referred to our service Benefits: Immediate start Condensed hours available Full time / contract - 6 month contract Competitive pay rate - From 32 p/hr No extra travel Supportive management Work within your contracted hours Requirements Please apply if you are a Qualified Social Worker with a minimum of 2 years experience in an adults disability/ mental health setting .
